University of Chicago Medical Insurance Plan (U-SHIP)

The university requires all registered students to be covered by medical insurance and automatically enrolls them in the university‚Äôs student group insurance plan. Students may however request a waiver of participation in the university plan on the basis of alternate comparable coverage from another source. For information about the plan including a description of the plan and the procedure and online forms for enrolling or requesting a waiver of participation please visit student insurance.

Immunization Compliance

A second health-related obligation to which nearly all students are subject is compliance with an Illinois law requiring new students to present proof of immunity from German measles, measles (two shots required), mumps, and tetanus/diphtheria (three shots required for international students). The Student Care Center notifies all new students of the requirement and provides instructions for compliance. Forms will be mailed to all incoming students and are available to be downloaded on the web at Student Immunization Requirements. Forms must be returned by mail or in person, they cannot be returned electronically.
